The other thing I have used is a google spreadsheet. I have to fill in the columns before going to bed.
Did I take my meds: yes or no
Exercise: fill in how much
Did I put myself down? good or bad
Did I "feel" like eating or was I hungry? describe a bit if I ate because I was upset, etc.
At least, that's the format I'm going to use. I don't know how long it will last.
